Stress incontinence and low testosterone levels, what relationship?

Nell ‘ stress incontinence may also play a role testosterone . At its low levels, researchers from Massachusetts General Hospital in Boston (United States) have associated an increased risk of suffering from this form of urinary incontinence. We talk about it with Dr. Rodolfo Hurle , urologist at Humanitas.

Stress urinary incontinence is characterized by involuntary leakage of urine following physical exertion ; in that of urgency, the losses occur when the need to go to the bathroom arises. Then there are mixed forms of incontinence.

The researchers started from studies already conducted on experimental models. Testosterone administration was seen to be associated with hypertrophy of the pelvic floor muscles with improved incontinence symptoms. The levator muscles of the anus, included in this muscle band, in fact have androgen receptors such as testosterone.

Low testosterone levels and muscle tone

To evaluate the correlation between testosterone levels and incontinence, researchers analyzed data for 2123 women who had taken part in a previous study. Participants were divided into four sets based on testosterone levels. Low levels were significantly associated with stress and mixed incontinence: participants with the lowest testosterone levels were more likely to report stress incontinence than women in the other three groups. The results of the study were presented at the latest congress of the American Urological Association.

«Testosterone, the male sex hormone , is present in variable proportions also in the female gender and performs some functions; among these the maintenance of the muscle tone of the pelvic floor seems to be among the main ones, as confirmed by the study in question В», explains Dr. Hurle. «A decrease in serum testosterone levels in women could result in a decrease in the tone of the urethra’s muscle sphincter and cause urine loss as a result of sudden physical exertion.»

Why is the association valid only with stress incontinence?

В«The mechanisms that determine» urgency «incontinence are different from the previous form. In this case it is the bladder muscle, an autonomous «smooth» muscle, therefore different from the «striated» muscle of the pelvic floor, which contracts independently of the efforts that the patient makes to keep the urine; the bladder muscle generates a pressure that exceeds that present in the urethra causing urinary loss «.

Could one think of testosterone therapy for women with incontinence? «The use of testosterone-based comprar tamoxifen citrate therapy in women has been proposed several times for multiple diseases,» recalls the expert. В«The results of the study presented at the Congress of American Urologists is attractive however, before starting a clinical trial, it will be necessary to weigh carefully the pros and cons that such therapy may have. Today we already have some molecules, such as duloxetine, which are used in the treatment of stress urinary incontinence in women, «concludes Dr. Hurle.
